The Assessment/Intake is the central point of admission to all programs. Screening and an assessment are provided to all individuals seeking care for themselves and or family members. Patients are assessed and referred to appropriate services within the organizational system of care. Services are provided on a walk-in basis or by appointment at our main location. We accept Medicaid and Medicare, and most Medicaid managed care plans and private insurances. A sliding fee scale is available for self-pay clients. At intake, you will need to bring proof of income, a government issued identification, and a list of all current medications.
CASE MANAGEMENT
Case managers work with pediatric and adult patients to ensure continuity of care through an assessment, programmatic monitors, personalized service plan, and linkages to community resources that meet health, economic, housing, and other social needs.
COMMUNITY RESIDENTIAL
24 hours, 7 days a week supervised residential services for adults unable to live independently in the community as determined by their physician and treatment team. Services are directed to patients who are at risk of homelessness and or hospitalization because of a limited ability to manage their living environment and physical and or behavioral health without supervision.
