Patients benefit from the full integration of our behavioral health program with our medical services as a Patient Centered Medical Home. A licensed clinical social worker is a member of every patient’s healthcare team at Greater Portland Health, and their role is to promote patients’ holistic health and deliver quality patient care by promoting health equity, diversity, and inclusion.

Clinical social workers provide evidence-based interventions for patients suffering from social/emotional issues such as stress, sad and anxious moods, marital issues, psychological trauma, and substance abuse issues. As needed, information and referrals to more intense treatment programs as well as substance abuse assistance are given.
