Case managers at New Hope work with clients to create individual service plans that serve as a roadmap for each person’s “next steps”—such as transitioning to residential rehabilitation, beginning an outpatient medication-assisted treatment maintenance program, or other transitional care—as well as a plan for each client’s new life in recovery. Personal responsibility and self-discipline are emphasized in New Hope’s philosophy and treatment program, while getting help from our qualified counselors.
